After marpe PLACEMENT Care Guide

You’ve just had your Miniscrew-Assisted Rapid Palatal Expansion (MARPE)/MSE II appliance inserted. This guide will help you know what to expect, how to care for it, and when to reach out if something isn’t right.

What You’ll Feel at First

  • Mild discomfort, soreness or pressure in the palate or around the screws is normal for 3-5 days.
  • Swelling of gums or slight irritation near miniscrew sites.
  • More saliva than usual and possibly speech sounding a bit different.
  • Sensation of tightness or fullness as the palate begins to expand.
  • Take painkillers as prescribed (e.g., ibuprofen or acetaminophen) if there’s discomfort.
  • Gargle with cold water or rinse mouth with lukewarm salt water.

  1. Brush gently, especially around the screws and appliance, using a soft toothbrush.
  2. Use a soft interdental brush to clean between appliance and teeth. Waterfloss can be a good alternative!
  3. Antiseptic mouth rinse (e.g. prescribed chlorhexidine or other) to reduce risk of infection.
  4. Rinse after meals to remove food debris.
  5. Avoid mouthwashes with alcohol (if sensitive).
  • Stick to soft, easy-to chew foods for the first few days: soups, mashed potatoes, yoghurt, smoothies.
  • Avoid hard, tough, crunchy, or sticky foods (nuts, hard candy, ice, crusty bread) that can stress the appliance or irritate tissues.
  • Cut foods into small pieces, chew slowly.
  • Stay hydrated; cold drinks can help soothe soreness.

Most patients adjust well to their MARPE, but if you notice any of these signs, please contact the clinic right away:

  1. Severe pain that does not improve after a few days or suddenly worsens.
  2. Bleeding that won’t stop or starts without brushing or touching.
  3. Swelling, redness, or pus around the miniscrews.
  4. Bad smell or taste coming from around the appliance.
  5. Loose miniscrews — if they feel wobbly or moving.
  6. Appliance feels broken or bent.
  7. Fever, facial swelling, or difficulty swallowing/breathing (rare but urgent).

  1. Brush gently twice daily + use interdental/proxy brush.
  2. Rinse after meals; saltwater rinses can help.
  3. Soft diet for a few days; avoid hard/sticky foods.
  4. Do activations only as shown and scheduled.
  5. Call us for severe pain, signs of infection, or if a screw is loose.
  6. Attend all follow-ups.
