Non-Extraction Solutions for Orthodontics

At MyBracesClinic, we understand that healthy teeth are precious and should not be removed unnecessarily, especially when there are modern, innovative approaches to achieving beautifully aligned smiles without resorting to tooth extraction. Traditionally, some orthodontic treatments involve removing teeth to create space, but non-extraction methods focus on preserving natural dentition while achieving optimal results.

Space is needed to resolve dental crowding/ crooked teeth, midline deviations, large overbites, and underbites. Without extractions, how can our Orthodontic team create the space to solve dental and bite problems?

Arch expansion:

Widening of the dental arches gently and efficiently using various modern techniques such as Invisalign and Damon braces. Our Orthodontic Team believes that the days of the fixed metal expander jack-screws are past. These bulky, 1-dimensional expanders are uncomfortable, limited in range, and require parents to turn the screw actively. No thanks! 

Distalisation:

Lengthening the dental arches via sequential distalisation basically means moving the molars and premolars backward to create extra length and space in the arch. Distalisation staging is customised based on the needs of the patient.  Our experienced Orthodontic Team will also check for obstacles that need to be cleared (eg: wisdom teeth) and plan anchorage for distalisation to be effective and efficient. For challenging cases like severe underbite treatment without jaw surgery, Temporary Anchorage Devices (TADs) may be indicated to boost anchorage. Invisalign is a practical, elegant, and painless way of distalisation to create space.

Teeth uprighting and rotations:

Our Orthodontic Team is trained to see ‘hidden’ spaces that can be unlocked in a crooked smile. Teeth that are abnormally positioned can reveal ‘hidden’ spaces when corrected and must be factored into space analysis to avoid dental extractions. 

Archform correction:

Collapsed, skewed, and deformed archforms often reveal spaces when these abnormalities are corrected.

Dentofacial Orthopedics or Growth Modification:

By tapping the growth potential of a child or teen, our Orthodontic team can manipulate jaw growth to reduce the ‘overbite’ and underbites discrepancies and the need for dental extractions. 

Interproximal Reduction (IPR):

Reshaping large teeth can be a useful method to: 

  • Gain space (limited to 0.5mm) to relieve crowding and correct midlines
  • Reshape teeth for better aesthetics
  • Reduce interdental ‘black triangles’
  • Improve contact points between teeth

Surgical Traction for impacted teeth:

Sometimes, adult teeth can get ‘stuck’ under the gum or impacted. This does NOT mean that extraction is the only way out. Our Orthodontic Team and Oral Surgeons can move the affected tooth back into the mouth. The surgeon will surgically bond a traction chain to the impacted tooth, so our Orthodontic Team can start traction and guide it into its correct position.

Careful assessment of the position of the impacted tooth, inclination of the affected tooth, location of the root apex, and development stage of the root is important for planning and staging.

It is crucial to approach non-extraction solutions for orthodontics safely and efficiently. Not all patients will benefit from non-extraction solutions and not all patients are suitable.

Extra-Oral Considerations:

Our Orthodontic Team will evaluate the facial profile, lip profile, nose: lip angulation, and lip: chin angulation to assess the suitability of a non-extraction approach. 

The non-extraction approach helps to increase lip support and is suitable for patients with thin lip profiles. 

Intra-Oral Considerations:

Non-extraction solutions need a healthy gum and jaw base to support extra tooth movement. Teeth must be housed comfortably in the jaw bone after treatment. Teeth mustn’t be pushed beyond their alveolar bone housing to achieve the goals of non-extraction therapy. Gum assessment and a 3D X-ray (CBCT) may be needed for complex non-extraction tooth movement.
